For your convenience and to shorten your wait time, please complete the patient information and medical history through the Portal, which we require for your examination. We ask that you complete the questions in their entirety (every question must be completed).
Bring a photo ID and current insurance cards.
Arrive 15 minutes early to your appointment to register if your Portal registration is already completed.
Arrive 30 minutes early if you have not completed your paperwork.
If you have been treated by another orthopaedic physician or pain management specialist, you must bring all records and x-rays films on your initial visit.
If you had x-rays or other images such as MRI's, Bone Scans, etc. taken at another facility, contact them immediately to make arrangements to pick these up and bring them to your appointment. If you are unable to obtain these records, please contact our office to reschedule with 24 hour notice. If you arrive without these films / records, your appointment will be rescheduled.
If the patient is under the age of eighteen, a legal guardian must be present. If other than legal guardian, a notarized letter must accompany adult attending appointment with minor. The patient information form must be completed by adult attending appointment and is responsible for the bills incurred.
If you have a power of attorney, we require a copy of the documentation for our files.
We require a 24-hour cancellation notice. If you do not notify us within 24 hours before your appointment, there will be a charge of $ 25.00. This fee must be paid before any future appointments are made.
Before leaving your appointment, make sure you have a work slip for your employer if required and your prescription(s).
If you need a routine prescription refill, please have your pharmacy request your medication(s) via E-Script. If for some reason the E-Script system is down, they can fax the request to (407) 654-4937.
Please make your request before your run out, as it can take anywhere from 24 to 72 hours to obtain your doctors approval.
If you are having difficulties with your current medication(s), you can contact us thru the Patient Portal or by submitting the Contact Us Form.
We do not release our original X-ray films. There is a fee to copy X-rays which must be paid in advance via money order, cashiers check or cash. Please contact Ext. 226 to find out the cost of copying your X-rays.
