Office Information: About Your Visit

Appointments

For your convenience and to shorten your wait time, please complete the patient information and medical history through the Portal, which we require for your examination. We ask that you complete the questions in their entirety (every question must be completed).

Bring a photo ID and current insurance cards.

Arrive 15 minutes early to your appointment to register if your Portal registration is already completed.
Arrive 30 minutes early if you have not completed your paperwork.

If you have been treated by another orthopaedic physician or pain management specialist, you must bring all records and x-rays films on your initial visit.

If you had x-rays or other images such as MRI's, Bone Scans, etc. taken at another facility, contact them immediately to make arrangements to pick these up and bring them to your appointment. If you are unable to obtain these records, please contact our office to reschedule with 24 hour notice. If you arrive without these films / records, your appointment will be rescheduled.

If the patient is under the age of eighteen, a legal guardian must be present. If other than legal guardian, a notarized letter must accompany adult attending appointment with minor. The patient information form must be completed by adult attending appointment and is responsible for the bills incurred.

If you have a power of attorney, we require a copy of the documentation for our files.

We require a 24-hour cancellation notice. If you do not notify us within 24 hours before your appointment, there will be a charge of $ 25.00. This fee must be paid before any future appointments are made.

Before leaving your appointment, make sure you have a work slip for your employer if required and your prescription(s).

Fees & Payments

For office services: Methods of payment: Cash, Cashier's Check, Check, Money Order, MC, Visa, Discover and American Express.
 
For down payment of surgeries: Method of Payments: Cash, Cashier's Check, Money Order, MC, Visa, Discover and American Express.
 
Methods of payment for disability forms, FMLA, copies of medical records and/or x-rays: Cash, Cashier's Check or Money Order.

Prescriptions & Renewals

If you need a routine prescription refill, please have your pharmacy request your medication(s) via E-Script. If for some reason the E-Script system is down, they can fax the request to (407) 654-4937.

Please make your request before your run out, as it can take anywhere from 24 to 72 hours to obtain your doctors approval.

If you are having difficulties with your current medication(s), you can contact us thru the Patient Portal or by submitting the Contact Us Form.

Medical Records and X-rays

Medical Records and X-rays are a permanent part of your medical record. We do not release our originals. These are not copied on demand or at the time of office visits. You must submit a written request in advance along with a photo ID. We use ACOPI, a copy service, to copy our records, and they will contact you regarding the fee to copy your medical records.

We do not release our original X-ray films. There is a fee to copy X-rays which must be paid in advance via money order, cashiers check or cash. Please contact Ext. 226 to find out the cost of copying your X-rays.

Disability and Family Leave Act

Disability and Family Leave Act Forms are completed in work rotation. Present these to the front office staff at time of visit with your portion completed to its entirety. When the doctors attempt to complete these during a visit, they generally do not complete these to their entirety and many times, the disability company or employer will have them resent for proper completion. If you ask the doctor to complete these forms during a visit, an answer can be missed, which could cause a delay in your disability payment. These forms can take anywhere from 3 to 10 days depending on the information needed from your medical record. Disability Forms are $25.00 each and Family Leave Act Forms are $25.00 each. We accept only money order, cashiers checks or cash for these services. The patient must complete their portion, date and sign, and bring or mail to the office the fee per form.
